A dental implant is an artificial tooth root, typically made of biocompatible materials, that is surgically placed into the jawbone to support a dental prosthesis. In prosthodontics, implants are a cornerstone for restoring missing teeth, ranging from single crowns to full-arch fixed bridges and removable overdentures.
Core Components of an Implant System
An implant product is not a single piece but a system, generally composed of three main parts:
- Implant Fixture (Implant Body): The screw-shaped component inserted into the bone. It osseointegrates (fuses) with the jawbone over time.
- Abutment: A connector piece attached to the implant fixture. It protrudes through the gum and serves as the anchor for the final restoration.
- Prosthesis (Superstructure): The visible, custom-made tooth replacement - a crown, bridge, or denture - which is fixed onto the abutment.
Some systems also offer one-piece implants where the fixture and abutment are a single unit, often used in specific overdenture cases.
Common Materials
- Commercially Pure Titanium (CP Ti) and Titanium Alloys (Ti-6Al-4V): The gold standard due to excellent biocompatibility, strength, and proven osseointegration.
- Zirconia (Ceramic): A metal-free, tooth-colored alternative prized for aesthetics and in patients with metal sensitivities or thin gum tissue. Zirconia implants are a growing segment of the product range.
Design and Surface Technology
Modern implant products are differentiated by their design features and surface treatments that enhance bone healing and stability:
- Macro-design: Tapered, cylindrical, or hybrid body shapes. Thread patterns vary (e.g., self-tapping, double-lead threads) to achieve high primary stability.
- Surface Micro-topography: Most systems use moderately rough surfaces created by techniques like sandblasting with large-grit and acid-etching (SLA), anodization, or calcium-phosphate coating. These surfaces accelerate osseointegration.
- Connection Type:
- Internal Connection (e.g., Internal Hex, Morse Taper): Widely preferred for better force distribution, microbial seal, and connection stability.
- External Connection (e.g., External Hex): The traditional design, still in use.
- Platform Switching: An abutment narrower than the implant platform, designed to preserve crestal bone and improve soft tissue aesthetics.
Product Categories by Prosthetic Indication
From the prosthodontist's perspective, implant products are selected based on the type of prosthesis planned:
- Implants for Fixed Restorations: These must accommodate screw-retained or cement-retained crowns and bridges. Products include bone-level (placed flush with bone, allowing customized emergence profile) and tissue-level implants (a smooth collar sits above the bone, simplifying impression taking).
- Implants for Removable Overdentures: Designed to retain removable dentures. Key products here are narrow-diameter/mini-implants and standard implants with special overdenture attachments:
- Ball/Socket Attachments: A ball abutment on the implant snaps into a housing in the denture.
- Locator® Attachments: A popular, low-profile system with interchangeable retention inserts for varying retention needs.
- Bar Attachments: A customized metal bar connects multiple implants, and the denture clips onto the bar for excellent stability.
- Full-Arch Fixed Solutions (e.g., All-on-4® Concept): Products for this protocol often include tilted implants, angled multi-unit abutments, and immediate-loading designs. This allows a full set of fixed teeth to be supported by only four implants, often with a provisional prosthesis delivered on the day of surgery.
Role of Implant Products in Complete Denture Restoration
For edentulous patients, implant products transform traditional dentures. An implant-supported overdenture uses 2-4 implants to provide retention and stability, dramatically improving chewing function and comfort compared to conventional dentures. The choice of attachment system (Locator, ball, or bar) depends on the patient's anatomy, manual dexterity, and budget.
